Chronic Disease Prevention and Management


Committed to promoting healthy living for all, the Community Financial Management Program offers a variety of activities for those who suffer from or at risk for developing chronic diseases.

FAMILY HEALTHY BEGINNINGS INITIATIVE

All residents that are income eligible should apply to the Supplemental Nutrition Assistance Program, you can apply online or in person


To apply online:


  • Go the Virtual Gateway Online Application for Services page, check the box for SNAP, and click continue.
  • Read the instructions carefully as eligibility requirements can change.
  • You will see your application number, and you must select a password that should be easy to remember.
  • Fill out the online application and submit it.
  • The Department of Transitional Assistance (DTA) will contact you, mostly by a phone call and a letter, to let you know what documents you need to provide to complete the process of your application.
  • You will be assigned a Social Worker to take care of your case, if you have any questions contact this person directly, her/his name and telephone number will be in the letter that the DTA will send to you.
  • It is very important to send the required documents to the DTA as soon as possible, they can be mailed of faxed.
